Where you may require treatment or onward care, or in certain circumstances where our system determines you may need additional support (for example if we believe you may be at risk) our partners or SH.UK will make direct contact with you.
If we are unable to reach you on your mobile number after multiple attempts, we may send a letter to the address you provided. If we still cannot make contact, we may need to reach out to your GP. Please be assured that all communications will remain private and confidential. To avoid any issues, please ensure your contact details are kept up to date.
